Definition of Defect

Anything about a Product that is seen as ‘wrong’ by a Stakeholder; defects usually result in a new Item being added to the Backlog;

A bug, failure, flaw or error in an application or program that results in unexpected, incorrect or unintended ways.

Examples

A second major pattern of failure is a team not seeing any impediments, yet they have many open defects that are not quickly repaired.

Performance testing (this may need to be done in a test lab the Team doesn’t control) to find defects and holes to be plugged.

Using the extreme programming technique of 'pair programming', the rate of defects is lower. Thus, it can be hypothesized that pair programming is an effective technique to reduce new defects when either new code or existing code is worked on.
